445 - Investigating unusual root resorption following fixed orthodontic treatment- a case report

Introduction: Pathological root resorption during orthodontic treatment is not uncommon and there are several factors which may influence the extent of resorption such as duration of treatment, magnitude of force and systemic diseases in the patient. Case: This case will describe a teenage boy who was referred by his dentist to the paediatric dental department, 17 months after having his fixed orthodontic appliance de-bonded. The reason for the referral was an incidental finding of root resorption of several permanent dentition seen on bitewing radiographs taken during a routine dental examination, post-orthodontic care. There was no evidence of root resorption prior to bonding the fixed appliance. Findings: On initial examination, no clinical abnormalities were detected. However, an orthopantomogram (OPT) radiograph revealed severe resorption of multiple roots including the distal root of the mandibular left second molar. A radiology report from a dental radiology specialist suggested that these findings are mostly consistent with resorption due to orthodontic treatment. However, this does not explain the resorption seen on the distal root of the mandibular left second molar, which was unlikely to have been bonded during the patient's orthodontic treatment. Further investigations: The patient was referred to his GP for blood tests to investigate for systemic causes e.g. hyperparathyroidism, hypoparathyroidism. His parathyroid levels were found to be higher than the normal range. He will be reviewed on clinic to discuss whether further investigations and treatment are required, with respect to these results. Clinical relevance: This presentation will display our radiological and clinical findings and shed light on differential diagnoses for the unusual pattern of root resorption seen in this patient. It will also explore the relevance of radiographic assessment for patients during and after orthodontic treatment.
