430 - Audit: Surgical management of ectopic canines (2 Cycle)

Introduction: Impaction of canines is a problem frequently encountered by oral surgeons and orthodontists. The prevalence of impacted canines is 1-2%; with 85% of those being palatally displaced. The treatments that are offered in our unit are extraction of primary canines, surgical exposure and orthodontic alignment and surgical extraction of the ectopic canine. Aims: a) to evaluate the range of methods used to treat impacted canines b) to assess the outcomes and complications against the current guidelines Methodology: This 2 cycle audit was carried out retrospectively using 130 cases over 2011-2019 treated under general anaesthetic in secondary care. For each patient, the data collection included the initial referral to the hospital through to the completion of treatment and post-operative care. The three main areas studied within the patient notes were clinical examination, consent and treatment outcome. The set standard was based on the first cycle of data as 60% spacing information included; 85% palpation information included; 100% consent obtained; 100% warned of risk of potential damage to adjacent teeth for surgical extraction; 100% warned of risk of failure of treatment for surgical exposure; 100% overall successful treatment and 95% intra or post-operative complications. Results: The results of the second cycle of the audit were: 67% spacing information included; 80% palpation information included; 100% consent obtained; 36% consent for damage to adjacent teeth for surgical extraction; 52% consent for risk of failure of treatment for surgical exposure; 100% successful treatment and 93% treatment with no complications. Conclusion: The above figures highlight that the main issue is consent. Montgomery V Lanarkshire 2015 describes that all material risks should be disclosed to patients. An information sheet was implemented into the maxillofacial guideline and plan to re-audit in 6 months.
