418 - Case study:Re-RCT of a mandibular second premolar and its successive restoration with a partial coverage prosthesis

This case study looks at failed root canal treatment of a mandibular second premolar and its successive re-treatment and restoration with a partial coverage ceramic prosthesis. A 65 year old male presented with a previously treated LR5 and symptomatic apical periodontitis. Clinically, the tooth was restored with a large DO composite restoration. The tooth was managed orthograde with removal of the existing coronal composite restoration followed by endodontic re-root treatment of the tooth. Upon completion it was restored with a bonded partial coverage onlay/overlay prosthesis. Bonding was done under rubber dam using a resin based Rely-X cement. This case illustrates current methodology in partial coverage preparation designs and how the advancement of new bonding protocols have allowed the feasibility of partial coverage crowns. Partial coverage crowns offer an array of advantages over direct composite restorations without the destruction of full coverage crowns. In theory, preserving of this tooth tissue structure can prevent catastrophic failure of these teeth. The coronal margins also help prevent traumatic interference with the marginal gingivae, preventing recession. advantages of ceramic overlays over direct composite include: control of the occlusal morphology; wear resistance; no negative effects of polymerisation shrinkage and associated leakage or cuspal fracture risk. expected survival rates of such a prostheis is quoted at 90% in 10 year survival studies.
