339 - Orofacial granulomatosis or Crohn’s Disease? An unusual case history…Getting to the bottom of it.

This case discusses the presentation of MW, a 48-year-old male who presented to secondary care with a long-standing history of significant lip swelling and deteriorating gastrointestinal symptoms, where despite extensive investigations, a definitive diagnosis cannot be reached. It discusses the presentation, differential diagnoses and special investigations undertaken while under joint care with the Gastroenterology Department, as well as the decision to start a steroid-sparing immunosuppressant without a definitive diagnosis, after weighing up the risks and benefits. It highlights the challenges faced when clinical presentation and results of investigations do not agree and reflects on the effects this has on quality of life. It acknowledges the complexities of dual department management and highlights the importance of teamwork and communication between different specialties, as well as between the clinician and patient.
